As the centerpiece of the new payment reform system, Accountable Care Organizations (ACOs) are poised to enhance quality, improve efficiency and reduce costs. The question on everyone’s mind is how to integrate the components of care across the continuum, especially between the acute and post-acute care service areas. At this year’s Mid-Winter Leadership Forum we will take a closer look at the current thinking on managing care across the continuum and hospitals’ role in this new landscape. We’ll look at how effective strategies can enable top-quality care and still bend the cost curve, how revenue can be spread across the continuum of care through better coordination of acute and post-acute services.  With thought-provoking keynote presentations and a panel discussion that will highlight the dimensions of the care continuum, you won’t want to miss this year’s Mid Winter Leadership Forum.
